Waiver or Release of Liability:  I hereby certify that my child is in normal health and capable of safe participation in any of the City of Sibley Recreation Programs. I assume all risks and hazards incidental to the conduct of the Recreation Department programs, and I agree not to hold the City of Sibley, the Recreation Department, the S-O School District, employees, agents and sponsors responsible for any injury that may occur during the participation of Sibley Recreation Programs. I hereby authorize the Sibley Recreation Department to obtain medical treatment, for my child, in the event that the parent/guardian or emergency contact cannot be reached. I affirm that I am voluntarily allowing my child to participate in the City of Sibley Recreation Programs and acknowledge that there are inherent risks in participating that cannot be eliminated even when the greatest care is taken. I know, understand, and appreciate these inherent risks.
